As an RN Case Manager within the Housing Services team, you will serve as a vital bridge between clinical care and housing stability. This role exists to provide medically-related case management services to clients, acting as a conduit to their medical providers while delivering nursing support. You will collaborate with a multidisciplinary team to improve overall client function and produce better housing outcomes.
Your day will involve delivering health education and counseling that draws upon each client's strengths, resiliency, and motivation. You will perform clinical tasks such as nursing assessments, treatments, and medication adherence support, often conducting home and community visits independently. Working with clients who may be homeless, low-income, or experiencing significant mental health challenges, you will utilize a client-centered, non-judgmental, and trauma-informed approach. You will help clients develop integrated treatment plans using SMART goals and navigate crisis situations with a flexible, open-minded attitude.
