About the Role
As an RN Care Coordinator, you will serve as a vital link between patients, physicians, and interdisciplinary teams. This role exists to ensure seamless care experiences by coordinating patient care across the healthcare continuum. You will drive quality, safety, and continual improvement by developing and monitoring individualized care plans, conducting comprehensive assessments, and closing care gaps using evidence-based protocols.
In this position, you will work closely with specialists to facilitate referrals, manage transitions of care, and support safe discharges. Your day-to-day involves educating patients and families on treatment plans and self-care while utilizing EMR systems to document progress and track outcomes. By communicating proactively, you will help enhance patient access and satisfaction within our community.
