About the Role

As a Family Nurse Practitioner (FNP), you will provide primary and preventive healthcare services to patients aged 2 and older, focusing on overall wellness, disease prevention, and chronic condition management. This role is designed for a clinician who can work autonomously while seamlessly collaborating with physicians and an interdisciplinary team. You will be the driving force behind assessing, diagnosing, treating, and educating patients to improve their long-term health outcomes.

In this position, you will not only manage acute and chronic illnesses but also serve as a mentor and clinical guide for nursing staff, medical assistants, and other healthcare professionals. You will play a key role in training and onboarding new team members, ensuring that best practices in treatment and follow-up care are upheld across the board. Your day will involve conducting comprehensive health assessments, prescribing medications, and coordinating care with specialists to ensure continuity for our members.

Beyond clinical duties, you will actively participate in community health outreach and wellness programs, contributing to quality improvement initiatives and the implementation of evidence-based practices.
