As a Primary Care Assistant-LPN, you will be an integral member of our outpatient clinic care team, supporting primary care providers in delivering patient-centered care. This full-time, daytime role focuses on health promotion, prevention, and chronic disease management for patients across the lifespan. You will bridge the gap between clinical administration and direct patient interaction, ensuring a seamless experience from intake to follow-up.
In this position, you will work closely with physicians, nurse practitioners, RNs, and medical assistants to support care plans and improve outcomes. Your day will involve engaging directly with patients and families to provide education on medications, treatment plans, and self-management strategies, all while maintaining Mercy's high standards for compassion, quality, and safety. You will also play a key role in care coordination and clinical triage, responding to patient questions and routing urgent concerns to the appropriate team members.
