This position serves as a vital support pillar for individuals residing in Residential Waiver Group homes. The core mission is to ensure the well-being and progress of residents with intellectual disabilities through comprehensive, personalized care planning. You will be responsible for conducting initial and annual assessments, developing robust Individual Support Plans (ISPs), and dynamically updating them as needs evolve.
In this role, you will act as a bridge between care implementation and strategic oversight. Your day-to-day involves tracking and monitoring the execution of support plans, utilizing strong communication skills to collaborate with direct care staff. A significant portion of your time will be dedicated to training staff on the effective implementation of these plans, ensuring consistency and quality of care across the catchment area.
