As a Community Services Coordinator, you will serve as a vital link between our Direct Support Professionals, the individuals we support, and the broader community. In this role, you will lead a team of Direct Support Professionals, ensuring they are trained, scheduled, and supported to deliver high-quality care. Your day will involve balancing office-based administrative duties—such as reviewing Medicaid Waiver documentation, managing schedules in Therap, and preparing for IPC meetings—with significant time spent in the community and at service locations.
You will be responsible for monitoring person-served plans of care, ensuring compliance with health and safety standards, and advocating for the rights of individuals with disabilities. This position requires a flexible schedule, as you will often need to work evenings, weekends, and holidays, and share in an on-call rotation to meet the dynamic needs of our community. Whether coordinating leisure activities, managing crisis situations with a calm demeanor, or ensuring facilities meet safety inspections, your goal is to create a positive, inclusive atmosphere where everyone thrives.
