As a Case Manager, you will serve as the primary point of contact for members receiving Long Term Services and Supports (LTSS) through Medicaid or State Funded programs. This role is designed for professionals who thrive in a fast-paced environment and are passionate about advocating for vulnerable populations, including those with disabilities and the elderly.
In this position, you will provide specialized, ongoing case management by monitoring services, maintaining open communication with members and providers, and coordinating resources to meet unique needs. You will act as an expert in eligibility assessments, service plans, and prior authorization requests, ensuring that every member receives the safe and healthy support they require.
Your day will involve a blend of administrative precision and compassionate fieldwork. You will conduct in-person eligibility and monitoring visits at members' residences, facilitate team meetings with care teams (primarily for IDD waivers), and collaborate with providers to ensure service plans are accurate and timely. Whether responding to complex needs, managing incident reports, or guiding members through the complaint and appeal process, you will represent RMHS's values of empathy, professionalism, and ethical conduct in every interaction.
