As a Case Manager, you will serve as a vital link between exceptional care and the transformative impact on each patient's journey. You will manage resources and coordinate patient care from admission to post-discharge, overseeing interdisciplinary plan-of-care decisions to ensure the well-being of individuals in your community.
Working with an interdisciplinary team, you will guide treatment plans based on patient needs and preferences, facilitate weekly team conferences, and monitor the patient experience regarding quality, timeliness, and service appropriateness. Your day-to-day involves establishing discharge plans, conducting assessments, and ensuring compliance with regulations and payor requirements.
