About the Role

As the Director of Case Management, you will serve as a key member of our leadership team, combining fulfilling career opportunities with the chance to make a meaningful difference in the well-being of your local community. In this position, you will lead a team and oversee the day-to-day operations and management of the Case Management department. You will act as a patient and family advocate, ensuring services are delivered to meet the unique needs of patients and their families. Your leadership will guide the interdisciplinary plan of care and discharge planning process, with a central focus on census management, patient care outcomes, and key care indicators. You will also build strategic relationships with insurance companies, self-insured employers, case management firms, and other healthcare networks. Beyond operational oversight, you will provide guidance and support to Case Managers and staff, fostering a vibrant community of individuals passionate about their work.
