As a Registered Nurse – Clinical Case Consultant in our CalAIM Enhanced Care Management (ECM) program, you will serve as the clinical cornerstone of a multidisciplinary team. Your mission is to act as a catalyst for better health outcomes among the state's most vulnerable populations—specifically those experiencing homelessness, living with serious mental illness (SMI), substance use disorders (SUD), or multiple chronic conditions.
In this role, you will ensure that every care plan is clinically sound, culturally responsive, and designed to help members live healthier, more stable lives. You will provide clinical oversight to Care Coordinators and Community Health Workers, conducting case consultations to ensure safety and alignment with member goals. Your day-to-day involves bridging service gaps by collaborating with providers, health plans, and community organizations to support seamless transitions of care from hospital to home. You will also lead training initiatives to empower non-clinical staff on chronic disease management and medication safety, while analyzing outcomes to drive program innovation and reduce avoidable emergency visits.
