As a Manager in Case Management, you will act as a vital liaison between patients, families, physicians, the healthcare team, and community agencies. In this role, you will establish, implement, and monitor best practices and clinical standards to assure the quality of patient care and the financial integrity of the organization. You will lead interdisciplinary efforts to address complex discharge issues, coach staff in finding alternative levels of care, and collaborate with direct care providers to rectify variances in patient processes and systems. Your day will involve developing operational budgets, maintaining professional relationships with community providers, and ensuring regulatory compliance through robust documentation and policy development.
