About the Role
As a Manager of Care Management, you will lead a multidisciplinary team dedicated to overseeing care coordination, utilization management, and safe transitions of care. This role exists to bridge the gap between clinical needs and operational efficiency, ensuring that patients receive seamless, patient-centered support throughout their healthcare journey.
In this position, you will mentor staff, collaborate closely with physicians, nursing, social work, and ancillary teams, and leverage data to refine workflows. Your day-to-day involves developing care management protocols, monitoring performance metrics, and engaging patients and families to address barriers to access and adherence. You will foster a supportive environment that encourages professional growth while maintaining strict adherence to regulatory and accreditation standards.

Requirements
- verifiedRegistered Nurse (RN) license
- verifiedBasic Life Support (BLS)
- verifiedCase Management Certification (CCM or ACM) preferred
- verifiedUtilization review experience preferred
- verifiedLeadership or management experience in care management
Nice to Have
Case Management Certification (CCM or ACM). Utilization review experience.
