As a Care Manager RN at Providence St. Vincent Medical Center, you will provide essential services to help progress patients through the continuum of care in an acute care setting. Your role involves screening for post-acute needs, transition planning, care coordination, patient education, and making community care referrals. You will develop care plans in accordance with regulatory requirements, collaborate with the continuum care team, and continuously monitor plan effectiveness to reassess the need for Care Management services.
This is a Per Diem/On-Call position working Day Shifts (8:00 AM – 4:30 PM) with 8-hour shifts.
