As an RN Care Manager in the acute care setting, you will provide a variety of services to help progress patients through the continuum of care. This role exists to ensure seamless transitions and effective long-term outcomes for our most vulnerable community members.
Your day will involve screening for post-acute needs, transition planning, care coordination, and patient education. You will collaborate closely with the continuum care team to develop care plans in accordance with regulatory requirements, make community care referrals, and conduct ongoing monitoring of plan effectiveness. You will also reassess the continued need for Care Management services to ensure patients receive the right level of support at the right time.
