About the Role

This 13-week travel contract seeks an experienced Registered Nurse to serve as a Case Manager within a high-volume acute care setting. The role is designed to coordinate comprehensive patient care, perform utilization reviews, and manage discharge planning for a diverse population ranging from neonates to geriatrics. With an average daily census of approximately 300 patients, the Case Manager will work with a ratio of roughly 1:30, collaborating closely with interdisciplinary teams including physicians, therapists, and social services.

The day-to-day involves conducting patient needs assessments, developing individualized care plans, and ensuring compliance with regulatory standards such as CMS, HIPAA, and Joint Commission requirements. You will also facilitate the ordering of durable medical equipment and post-discharge services while maintaining documentation in the Epic EMR system. This position requires flexibility to float within the facility's scope to meet operational needs and may include weekend coverage based on facility requirements.
