RN Case Manager

About the Role

The Registered Nurse (RN) Case Manager serves as a vital support system, providing expertise through comprehensive assessment, planning, implementation, and evaluation of individual patient needs. This role is central to ensuring care coordination and effective discharge planning.

Working closely with bed management, the RN Case Manager addresses bed capacity issues in a timely and ongoing manner. A key component of the position involves educating physicians and the broader care team on utilization review issues, including the appropriateness of admissions, levels of care, and external placements.

Under the direction of the Director of Case Management and Physician Advisor, this role is responsible for implementing and coordinating admission, discharge, and transition of care needs. The RN Case Manager adheres to departmental and organizational goals, objectives, standards of performance, and policies to continually assure quality patient care and regulatory compliance.
