This position serves as a vital leader within the VNS Health Hospice Care Program, managing, coordinating, and facilitating an interdisciplinary team dedicated to providing compassionate care to patients and their families, including after-hours support. The role exists to ensure the seamless implementation of clinical and quality improvement initiatives while maintaining strict compliance with all regulatory requirements.
In this capacity, you will champion an interdisciplinary approach to assess the medical, physical, social, emotional, and spiritual needs of patients. Your day-to-day involves managing departmental operations, including caseload and visit management, while working to achieve productivity and revenue targets. You will collaborate closely with Quality Improvement and Education teams to develop professional practice standards, participate in team meetings, and assist leadership in achieving hospice care goals through compliance and innovation.
Beyond clinical oversight, you will perform inherent managerial duties such as staff training, performance evaluation, budget development, and personnel decisions. This role requires a deep commitment to excellence in clinical practice and customer service, ensuring that support and direction provided to families align with the Hospice Care mission, objectives, and policies.
