The PACE Primary Care Practitioner (PCP) is a progressive, population-based health role dedicated to providing exceptional quality care to participants. This position is responsible for coordinating all aspects of care, from evaluation and treatment to health education and executing Age-Friendly care. You will champion the PACE model of care across the continuum, understanding that medical decision-making is always done in the context of overall goals of care.
As a member of an interdisciplinary team (IDT), you will collaborate closely with nurses, specialists, and other healthcare professionals to develop and implement comprehensive plans of care. Your day will involve engaging in advance care planning and palliative approaches, visiting participants in their homes or care settings, and ensuring clear, accurate documentation within the Electronic Health Record. You will serve as a vital link between participants, their families, and the broader healthcare system, fostering a safe and supportive environment for both patients and colleagues.
Candidates must meet specific legal and clinical requirements to practice within the organization. This includes holding a valid, unrestricted license as an Oregon-licensed Nurse Practitioner or Physician Associate, maintaining a current DEA certificate, and possessing a valid Oregon driver's license. All staff members with direct contact with participants must be legally authorized to practice in the state, act within their scope of authority, and meet a standardized set of competencies established by the PACE organization before working independently. Additionally, all staff must be medically cleared for communicable diseases prior to engaging in direct participant contact.
