About the Role
As a Nurse Practitioner or Physician Assistant at Monogram Health, you will be a key member of an integrated Care Team that includes a Registered Nurse and a Social Worker. This role is designed for skilled practitioners eager to make a tangible difference in the lives of patients who often struggle with multiple serious diseases, particularly Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD/ESKD).
In this position, you will deliver personalized, compassionate medical care directly in the patient's home or via a virtual environment. Your day will involve conducting comprehensive assessments, prescribing medications, ordering tests, and collaborating with the patient's Monogram physician. Beyond clinical duties, you will counsel patients and families on available benefits and programs to help them live healthier lives, coordinate care with primary providers and specialists, and facilitate the closing of care gaps through education and preventive monitoring.

Requirements
