The Targeted Case Manager serves as a vital link within our Integrated Health Care Clinic in Syracuse, NY, focusing on care coordination and resource linkage for clients. This role is designed to support individuals living with severe mental illness and/or substance use concerns, ensuring they receive comprehensive and tailored care to improve their overall well-being. You will play a key role in managing referrals for new clients and conducting outreach activities to assess and connect individuals with the specific services they need within the agency. Your work will involve collaborating with a treatment team to develop measurable, cost-conscious Plans of Care that address all areas of an individual's needs, not just health or mental health.
