The Personal Service Coordinator / Intensive Case Manager serves as a vital link for clients eligible for permanent housing through the Department of Health Services (DHS) Housing for Health (HFH) program. This role is designed to provide high-quality Intensive Case Management Services (ICMS) to individuals experiencing homelessness who face complex medical and behavioral health challenges.
In this position, you will adopt a client-centered approach, guiding individuals through every stage of the housing stabilization process. From initial outreach and screening to move-in coordination and long-term tenancy support, you will ensure clients receive the resources they need to maintain permanent supportive housing. You will act as a primary point of contact for housing subsidies, coordinate with DHS and partner agencies, and assist clients in accessing essential health, mental health, and substance use disorder services. The role requires a deep sensitivity to the unique barriers faced by this population and a commitment to helping them achieve meaningful, healthy lives in their communities.
