Care Management is a vital service dedicated to helping children with chronic physical or emotional issues obtain and utilize the medical, social, and community services they need to thrive. Our mission is to ensure these young members stay as healthy as possible, avoiding unnecessary emergency room visits by connecting them with the right resources.
As a Care Coordinator, you will serve as the essential link between children with chronic medical or behavioral health conditions and the comprehensive services they require. Working primarily in the field, you will assess risks, develop person-centered care plans, and coordinate appointments to optimize health outcomes. This role is a dynamic blend of office-based collaboration and community engagement, with 90% of your time dedicated to Non-Medicaid programs and 10% to Medicaid programs.
Your day-to-day involves traveling throughout New York City (Bronx, Brooklyn, Manhattan, and Staten Island) to meet clients in their homes, hospitals, clinics, and benefit offices. You will foster collaborative relationships with community providers, schools, and medical teams to create Individualized Plans of Care. Beyond clinical coordination, you will act as an advocate, helping families navigate educational services, housing, and entitlements while managing crises and responding to member needs during regular and after-hours periods.
We operate within a team setting that values interdisciplinary collaboration and knowledge sharing. The Jewish Board is committed to accessibility and inclusion; while our offices are ADA accessible, we recognize that community sites may vary. We provide agency equipment, including a cell phone, laptop, and hotspot, to support your hybrid work model, which currently requires a minimum of three office days and two hybrid days, though this may transition to 100% in-person based on program needs.
