As an After Hours Hospice RN, you will respond to patient needs during evenings, nights, weekends, and holidays—bringing peace of mind and compassionate care when it’s needed most. You will serve as the primary liaison between patients, families, physicians, and the hospice team during your shift, evaluating needs to guide clinical decisions and implement the plan of care.
This role requires you to work independently while ensuring continuity of care and coordination with the daytime team. You will provide direct patient care and support, responding promptly to urgent calls and symptom management needs. Your day-to-day involves collaborating with an interdisciplinary group including LPNs/LVNs and CNAs, while accurately documenting clinical visits and participating in quality assessment programs.

1+ years of hospice, home health, oncology, or palliative care experience. Familiarity with symptom management, triage, and after-hours home-based care. Prior experience in on-call or overnight nursing roles. Strong documentation and electronic medical records (EMR) skills.
