As an After Hours Hospice RN, you will respond to patient needs during evenings, nights, weekends, and holidays, bringing peace of mind and compassionate care when it is needed most. This is a 7-on/7-off position where you will serve as the primary liaison between patients, families, physicians, and the hospice team during your shift.
You will work independently while ensuring continuity of care and coordination with the daytime team. Your role involves evaluating patient and family needs to guide clinical decisions, responding promptly to urgent calls and symptom management needs, and supporting an interdisciplinary group including LPNs/LVNs and CNAs. You will provide direct patient care in homes, facilities, or wherever patients reside, ensuring that comfort, skill, and reassurance are delivered when it matters most.

1+ years of hospice, home health, oncology, or palliative care experience. Familiarity with symptom management, triage, and after-hours home-based care. Prior experience in on-call or overnight nursing roles. Strong documentation and electronic medical records (EMR) skills.
