About the Role

Are you a Registered Nurse who thrives on providing compassionate, whole-person care? We are seeking dedicated RN Case Managers who are passionate about walking beside their patients through every step of this sacred path. In this role, you will be more than just a caregiver; you will be a true advocate, companion, and comfort to patients and their families.\n You will make a true difference—one patient, one family, one compassionate moment at a time. Your day will involve developing individualized care plans, delivering attentive one-on-one support, and conducting thorough assessments of physical, emotional, and spiritual needs. You will collaborate with interdisciplinary teams to ensure comprehensive care delivery, ensuring that patients and families receive the highest standard of end-of-life care with skill and empathy.

This position operates Monday through Friday from 8:00 am to 5:00 pm, covering the area from Portland to Salem. Eligible employees may also participate in our Productivity Bonus Program, rewarding those who go above and beyond.
