About the Role

The RN Case Manager serves as a vital link in delivering compassionate, patient-centered care within home and long-term care settings. This role is dedicated to planning and managing a caseload of patients, ensuring that care aligns with the unique goals of both the patient and their family. Working collaboratively with an interdisciplinary team, the RN Case Manager adopts a holistic approach to navigate complex care journeys, from disease progression to end-of-life support.

In this position, you will be the coordinator of care, facilitating communication across various sites and ensuring timely responses to changing patient needs. The role requires a blend of clinical expertise and strong interpersonal skills to educate patients and caregivers on medications, nutrition, and hospice philosophy. Flexibility is key, as the position involves adapting to different care environments and supporting the broader community through education and advocacy.
