About the Role
We are seeking a compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary team. In this role, you will be the backbone of bedside nursing, delivering direct care that supports both rehabilitation and long-term care goals. You will collaborate closely with Registered Nurses, therapists, and physicians to implement individualized care plans, ensuring that every resident receives personalized attention.
Your day will involve administering medications and treatments, monitoring resident conditions, and documenting care in Electronic Medical Records (EMR). Beyond clinical duties, you will play a vital role in educating residents and families on health management and care plans. As a key member of our team, you will also supervise and support Certified Nursing Assistants (CNAs) to ensure high-quality daily care, all while adhering to strict infection control and safety standards.

Requirements
- verifiedActive LPN/LVN license in state of practice
- verifiedBLS/CPR certification
- verifiedIV therapy certification (preferred)
- verifiedExperience in skilled nursing or rehab setting (preferred)
Nice to Have
IV therapy certification. Experience in skilled nursing or rehab setting.
