Key Responsibilities
- check_circleProvide direct in-person patient care including admissions, routine visits, and death visits
- check_circleConduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs
- check_circleDevelop and revise the Hospice Plan of Care in collaboration with physicians and IDG members
- check_circleMaintain accurate, timely clinical documentation including assessments, care plans, and notes
- check_circleSupport administrative and compliance activities including workflow audits and survey readiness
- check_circleProvide leadership and guidance to field staff and conduct supervisory visits when appropriate
- check_circleAssess and coordinate patient and family needs during after-hours shifts
Requirements
- verifiedCurrent RN license in Tennessee
- verifiedGraduate of an accredited nursing program
- verifiedMinimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health
- verifiedCPR certification (in-person)
- verifiedValid driver's license, reliable transportation, and current auto insurance
- verifiedStrong knowledge of hospice principles and Medicare/Medicaid regulations
- verifiedProficiency with EMR systems (HomeCare HomeBase preferred) and Microsoft Office
