As a therapist at CenterWell Home Health, you'll play a vital role in helping patients regain strength, mobility, and independence—all from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you'll empower individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life.
As a Home Health Speech Language Pathologist, you will partner with patients, families, and healthcare staff to prevent, identify, and minimize the effects of speech, voice, language, hearing, and swallowing disorders. Your day will involve evaluating patients in home or facility settings, administering diagnostic tests, and utilizing professional judgment to direct therapeutic treatments. You will collaborate closely with the care team to develop and review Plans of Treatment and Care, ensuring a coordinated approach to recovery. Beyond clinical care, you will provide instruction on alternative communication systems and counsel patients and families, all while maintaining the highest standards of professional conduct and confidentiality.
