About the Role

As a Home Health Speech Therapist, you will partner with patients to address disorders of speech, voice, language, hearing, and swallowing. Your day will involve evaluating and treating these conditions to prevent, identify, and minimize their effects, ensuring patients can communicate effectively and swallow safely. You will collaborate closely with care teams, families, and physicians to develop and review Plans of Treatment, coordinate care, and manage discharge planning. This role requires maintaining the highest standards of professional conduct and confidentiality while providing instruction on alternative communication systems and training to patients and staff.
