CenterWell Home Health specializes in personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization. Our care teams include nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers, all working together to help patients rehabilitate, recover, and regain their independence.
As part of CenterWell, a Humana company, we are the nation's largest provider of senior-focused primary care and one of the largest providers of home health services. We are focused on whole-person health by addressing the physical, emotional, and social wellness of our patients. With more than 30,000 clinicians committed to putting health first, we offer stability, industry-leading benefits, and opportunities to grow your career through flexible scheduling, clinical certifications, leadership development programs, and career coaching.
As a Home Health Speech Therapist, you will play a vital role in helping patients regain strength, mobility, and independence—all from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you will empower individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life.
You will evaluate, direct, and provide speech/language pathology services to patients in the home or facility. Your day will involve utilizing professional skills to assess and treat disorders of speech, voice, language, hearing, and swallowing, while administering diagnostic tests and therapeutic treatments. You will collaborate with care teams to develop and review Plans of Treatment and Care, participate in discharge planning, and provide instruction to patients, families, and healthcare staff on alternative communication systems when appropriate.
