About the Role

This Social Worker position is designed to support patients and families navigating medically and socially complex situations with high psychosocial acuity. You will serve as a vital advocate, ensuring that the emotional and social needs of patients are addressed on an individual basis while facilitating quality care and education.

In this role, you will be the bridge between medical treatment and the patient's home life. You will conduct supportive visits to evaluate coping skills, family dynamics, and strengths, helping patients and families understand and accept medical recommendations. Your work involves intensive discharge planning, arranging post-operative care, and connecting families with community resources to ensure continuity of care. Whether providing counseling for anxiety related to a diagnosis, assisting with placement decisions, or coordinating care conferences, you will work collaboratively with the multidisciplinary team to alleviate fears and improve the quality of life for those we serve.
