About the Role
As a Social Worker at Avera, you will serve as a vital advocate for patients and families navigating medically and socially complex situations. This role exists to ensure that the emotional and social needs of patients are addressed on an individual basis, facilitating quality care and education throughout their journey.
You will be the bridge between medical treatment and the patient's home life. Your day involves assessing needs, counseling individuals and groups, and developing intensive discharge plans that prioritize safety and compliance. Whether arranging post-operative care, assisting with placement in assisted living facilities, or providing support during crisis situations, you will work collaboratively with a multidisciplinary team to alleviate anxiety and fears. You will utilize your expertise to connect patients with community resources, ensuring a seamless continuum of care that respects their unique backgrounds and strengths.
Hiring Process
Qualified candidates with a Master's in Social Work and an active South Dakota license are encouraged to apply. Please note that for this position, the South Dakota license is required upon hire.

Key Responsibilities
- check_circleInterview patients and families to assess home environment, health history, and coping skills
- check_circleEvaluate medical treatment plans and social service programs to develop personalized therapy programs
- check_circleArrange discharge planning, post-operative care, and placement into assisted living or nursing homes
- check_circleConduct individual, marital, family, and group therapies to address emotional and psychological needs
- check_circleCollaborate with multidisciplinary teams and case management to coordinate patient care and referrals
- check_circleMaintain patient documentation in the EMR and ensure compliance with department policies and safety standards
- check_circleProvide patient advocacy and support to alleviate anxiety related to illness, disability, and end-of-life issues
- check_circleAssist patients and families in understanding and following medical recommendations for compliance
Requirements
- verifiedMaster's in Social Work from an accredited program
- verifiedActive Social Worker license in the state of practice upon hire
- verifiedQualified Mental Health Provider (QMHP) license preferred
- verified1-3 years experience preferred
