About the Role
We are seeking a compassionate and highly motivated Licensed Social Worker (LSW) to join our team and serve a diverse population of seniors. This role is vital to our mission, as you will be the bridge between clinical care and the emotional realities of aging. You will provide a range of social work services tailored to individual needs, focusing on key areas such as aging in place, chronic illness management, caregiver support, grief counseling, and end-of-life care.
In this position, you will conduct comprehensive psychosocial assessments to understand the unique social, emotional, and physical landscape of each client. You will collaborate closely with seniors, their families, and a multidisciplinary team of physicians, nurses, and home health aides to develop and implement individualized treatment plans. Your work will involve providing therapy, advocating for clients' rights within the healthcare system, and educating families on available community resources.
