The Department of Care Coordination is dedicated to facilitating movement through the continuum of care for patients and their families. This team operates within the Emergency Department and Inpatient Units at the Downtown campus, providing a critical social work perspective to interdisciplinary activities.
The Social Worker III serves as a vital link between clinical care and the psychosocial needs of patients facing complex emotional or social challenges. In this role, you will provide direct social work services, including assessment, treatment, interventions, and recommendations. Your work ensures that patients and families receive comprehensive support as they navigate their healthcare journey.
You will develop and implement individualized plans of care responsive to patient concerns, health status, and available resources. This involves identifying patients requiring intervention, performing detailed assessments, and rating the severity of problems to guide treatment. You will provide a range of services, from helping with psychosocial functioning and adaptation to illness to facilitating protective services and post-hospital planning.
A significant part of your day involves collaboration. You will act as a resource for the interdisciplinary team, participating in patient care rounds and team conferences to advocate for the social work role and share insights on psychosocial issues. You will also develop and facilitate groups for patients and significant others, providing therapy, education, and support tailored to specific diagnoses and age-related needs.

Some familiarity with social work undergraduate education. Social work experience in a health care setting with the appropriate patient population and their families. Experience with identifying and navigating financial resources, including insurance coverage options, changes related to the federal and state health programs, and the disability process. Experience working with community resources, home healthcare, nursing facilities and other options available for patients in outpatient settings. Experience in a fast paced, high volume work environment. Care coordination experience or case management skills in a health care setting as part of an interdisciplinary team including experience utilizing an electronic medical record to assist with management of non-adherence, post-care follow-up and referrals to appropriate resources.
