The Child Welfare Case Manager is a vital link in our continuum of care, providing full wrap-around case management services to clients and families. This role is about more than just managing cases; it is about establishing genuine relationships while living out the agency's core values of courtesy, teamwork, and safety.
In this position, you will partner with children, parents, guardians, and community resources to develop collaborative case plans aimed at reunification or other permanency outcomes. You will act as the integrator of all services and supports for each child, ensuring trauma-sensitive transitions and coordinating everything from therapy and medical care to educational support. The role requires a high level of adaptability, as you will navigate irregular hours, on-call responsibilities, and travel throughout the state to meet families where they are.
