About the Role
As a Targeted Case Manager (TCM), you will serve as a vital link between clients and the resources they need to thrive. In this role, you will manage the case load of up to 25 clients, creating and implementing individualized care plans that address significant areas of each client's life. Your work involves coordinating services, monitoring progress, and facilitating meetings and monthly visits to ensure comprehensive support.
You will collaborate closely with therapists, community partners, and families to develop care plans that are reviewed every six months. A key part of your day involves connecting clients and their guardians with essential resources, attending treatment team meetings to offer strategic suggestions, and ensuring that all documentation meets Medicaid and agency requirements. This position requires a balance of administrative precision and empathetic engagement, often requiring you to work in diverse environments including client homes, schools, and community settings.
The Hiring Process
Candidates selected for an interview will be expected to demonstrate strong leadership, organizational, and conflict resolution skills. The process includes a review of qualifications, followed by interviews to assess cultural fit and competency in human services. Successful candidates must complete pre-employment screenings, including a background check, drug screen, and TB test.

Key Responsibilities

- check_circleReport incidents of abuse or potential abuse to appropriate authorities and the agency
- check_circleEnsure appropriate discharge planning is in place for each client
- check_circleCreate and implement individualized care plans addressing significant areas of client life
- check_circleObtain quarterly authorization for TCM services and maintain compliance with regulations
- check_circleCollaborate with therapists and community partners to develop and review care plans
- check_circleConduct face-to-face contacts with clients and complete TCM assessments for medical necessity
Requirements
- verifiedBachelor's Degree in human services or related field
- verifiedOne year of experience in community-based services postgraduate or Master's Degree in Human Services Field
- verifiedValid Driver's License and insurability
- verifiedProficiency with Microsoft Word, Excel, PowerPoint
- verifiedSuccessful completion of pre-employment drug screen
- verifiedSuccessful completion of background screening
- verifiedSuccessful completion of TB skin test or negative chest x-ray

- verified40 hours of annual training
