About the Role

As a Social Worker in our Senior Living and Short-term Rehab department, you will play a pivotal role in the holistic care of our residents. This position exists to ensure that the social and emotional well-being of our patients is prioritized alongside their medical needs. You will conduct comprehensive social service evaluations upon admission and develop tailored treatment plans that address medical, nursing, and psychosocial requirements.

Your day will involve collaborating consistently with an interdisciplinary team—including nurses, physicians, chaplains, and aides—to participate in regular plan-of-care meetings. You will provide ongoing patient visits to assess needs, monitor progress, and ensure alignment with established care goals. A key part of your role is acting as a vital link for patients and families by identifying community resources, liaising with external providers, and advocating for patient needs. Additionally, you will demonstrate expertise in dementia care, supervise volunteers or junior social workers, and coordinate with professionals across various care settings such as nursing homes, assisted living facilities, and hospitals.
