Based at Sanford Medical Center Fargo, this position serves as a vital member of the interdisciplinary team within the Emergency Department, with the flexibility to float to other inpatient areas. The role focuses on providing supportive services and counseling regarding healthcare and home care programs.
You will act as an advocate for patients, clients, residents, and their families, addressing social, emotional, and economic concerns to help them achieve or maintain an optimal level of functioning. This involves coordinating care plans, offering crisis intervention, and assisting families in understanding the complexities of medical situations. The role requires demonstrating a deep knowledge of human behavior and developmental stages while utilizing critical thinking and independent judgment to solve problems.
Depending on departmental needs, you may also provide social services for donors and transplant recipients. Success in this role requires effective communication skills to establish rapport with diverse communities and healthcare teams, as well as the flexibility to participate in special projects and care conferences.
