About the Role
As a Social Worker at Sanford Health, you will serve as a vital member of the interdisciplinary team, providing supportive services and counseling on healthcare and home care programs. Your mission is to help patients, residents, and their caregivers achieve or maintain an optimal level of functioning by coordinating complex care plans and addressing psychosocial challenges.
In this role, you will act as an advocate and a bridge between medical teams and the community. You will provide crisis intervention, assist families in understanding the complexities of medical situations, and develop comprehensive plans of care by connecting clients with essential social and human services resources. Depending on the department, you may also provide specialized social services for donors and transplant recipients. Your work requires demonstrating independent judgment, critical thinking, and a deep understanding of human behavior and developmental stages.

Key Responsibilities

- check_circleCoordinate and plan programs to address social, emotional, and economic concerns
- check_circleDevelop appropriate plans of care by obtaining resources from social and health agencies
- check_circleProvide referrals, information, and education regarding available programs and services
- check_circleRespond to suspected abuse, neglect, or violence in accordance with ethical policies
- check_circleCollaborate with interdisciplinary teams to support patient and family functioning
Requirements
- verifiedBachelor's degree in Social Work from an accredited curriculum
- verifiedBasic Life Support (BLS) certification within six weeks of employment
- verifiedLicensed Social Worker (LSW) or allowed credential in Minnesota
- verifiedLicensed Baccalaureate Social Worker (LBSW) in North Dakota
