As a coordinator for the Western Reserve Navigator program, you will plan and manage services to provide seamless coverage of all patient health care needs. You will serve as the primary point of contact, providing direct, professional, and knowledgeable interactions to patients, physicians, and referral sources. Your day will involve screening calls to determine medical and psychosocial needs, facilitating referrals to hospice levels of care, and coordinating care within the agency, between transdisciplinary team members, and with external partners.
In this role, you will oversee operational workflows by scheduling staff to support efficiency and patient satisfaction. You will educate allied health providers on program components, maintain medical files and documentation in the EMR, and generate reports for clinical staff. Additionally, you will coordinate admissions and discharges, manage service recovery, verify payor sources, and ensure continuity of care through after-hours communication.
