United Health Care Services is seeking a reliable and compassionate Transportation Provider to join our State Medicaid Supported Community Connection (SCC) program. The primary mission of this role is to transport program participants to and from various community activities, appointments, and services. The SCC program is dedicated to supporting individuals with disabilities in engaging with their communities and improving their quality of life.
In this position, you will be the vital link connecting participants to the world around them. Your day will involve safely transporting individuals to scheduled activities while ensuring their comfort and well-being. You will assist participants in and out of the vehicle as needed and maintain a clean, well-functioning vehicle. Effective communication with participants, caregivers, and program staff regarding schedules and potential changes is essential. You will also be responsible for maintaining accurate transportation logs in compliance with Medicaid requirements and responding to any transportation-related emergencies or issues in a calm, professional manner.
