Since 1996, Community Healthcare of Texas has been dedicated to providing compassionate hospice and palliative care services. Our mission is to support individuals living with serious illnesses while caring for their loved ones who provide daily support.
We are currently recruiting for an Advanced Practitioner Nurse to join our team. Under the direction of the Chief Medical Officer, you will collaborate with physicians and care team members to deliver high-quality palliative and hospice health care services to patients and families.
In this active practice, you will be responsible for documenting medical histories, tracking disease progression, and managing symptom control through close coordination with an interdisciplinary team. You will review and update written plans of care, consult with the Medical Director on complex cases, and establish relationships with community referral sources to facilitate program growth. This role requires navigating local, state, and federal regulations while ensuring comprehensive care for all payers.
This position operates Monday through Friday from 8:00 am to 5:00 pm. The role involves travel for patient visits, requiring a valid Texas Driver's License and proof of current automobile liability insurance.

Additional accreditation from an approved certifying body for advanced practice nursing (American Academy of Nurse Practitioners, American Nurse Credentialing Center, or HPNA Advanced Certification in Hospice and Palliative Nursing ACHPN). Minimum of 3 years of nursing experience, preferably at the APN level.
