We are currently recruiting for an Advanced Practitioner Nurse to join our team. Under the direction of the Chief Medical Officer, you will collaborate with physicians and care team members to deliver high-quality palliative and hospice health care services to patients and families.
In this active practice, you will be responsible for documenting medical histories, tracking disease progression, and managing symptom control. You will work closely with an interdisciplinary team to review and update care plans, ensuring patients receive appropriate measures for their end-stage disease process. Additionally, you will build and nurture relationships with staff and community referral sources to facilitate program growth, while participating in quality assurance and improvement processes.

Additional accreditation from an approved certifying body for advanced practice nursing (American Academy of Nurse Practitioners, American Nurse Credentialing Center, or HPNA Advanced Certification in Hospice and Palliative Nursing ACHPN). Minimum of 3 years of nursing experience, preferably at the APN level.
