The Primary Care Physician (PCP) is a leader within our organization, responsible for a panel of up to 450 patients. You will act as a health coach rather than just a consultant for sickness, helping patients set and achieve short and long-term health goals. Your work involves building trusting relationships to influence patient outcomes and simplifying care plans to make behavior change actionable.
You will lead a care team consisting of care promoters, facilitators, and coordinators, while collaborating closely with managers and medical directors. The role requires balancing independent judgment with strong collaboration across health care professionals, including hospitalists and specialists. You will champion a positive culture at your center and actively participate in recruiting patients and providers to support our growth.
