About the Role
The Primary Care Physician (PCP) at ChenMed is more than a consultant for sickness; you will act as a health coach dedicated to helping patients set and achieve short and long-term health goals. You will manage a panel of up to 450 patients, building trusting relationships that influence behavior change and improve outcomes.
In this role, you will simplify care to make it actionable for both patients and your care team. You will lead a collaborative team including care promoters, facilitators, and coordinators, while maintaining close partnerships with hospitalists and specialists to ensure seamless care for patients in the office, hospital, or home. As a leader within your center, you will champion a positive culture and actively participate in recruiting and center management.
