About the Role
As a Primary Care Physician (PCP) at ChenMed, you will serve as a health coach rather than just a consultant for sickness. You will build trusting relationships with a panel of up to 450 patients, helping them set and achieve short and long-term health goals. Your role focuses on accountability for outcomes, simplifying care to make behavior change actionable, and leading a dedicated care team.
You will work in an outcomes-focused, value-based environment where metrics like patient admissions, clinical gap closures, and patient experience are regularly measured. As a leader within the organization, you are expected to champion a positive culture of love, accountability, and passion alongside center leadership. You will function independently while collaborating closely with managers, medical directors, and care teams to ensure compliance with guidelines and promote patient health.
