The Primary Care Physician (PCP) at ChenMed is more than a consultant for sickness; you will act as a health coach dedicated to helping patients set and achieve short and long-term health goals. You will manage a panel of up to 450 patients, demonstrating accountability for clinical care, cost-effectiveness, and patient outcomes.
In this role, you will simplify care to make behavior change actionable for your patients and your care teams. You will lead a collaborative care team—including care promoters, facilitators, and coordinators—to ensure comprehensive management of acute and chronic illnesses in older adults. Whether patients are in the office, hospital, skilled nursing facility, or homebound, you will maintain active engagement and oversight of their care.
As a leader within your center, you will champion a positive culture, collaborate with managers and medical directors on compliance and quality management, and actively participate in recruiting patients and providers. You will work independently while maintaining strong collaboration with specialists, hospitalists, and other healthcare professionals to ensure the highest quality of care.
