As a member of the Case Management Team, you will apply your RN clinical expertise to drive care coordination and discharge planning. Your mission is to facilitate the delivery of quality, cost-effective, patient-centered care that spans the entire continuum—from pre-admission through the post-discharge phase. You will ensure care plans are tailored to meet individualized patient outcomes while monitoring services during acute hospital stays to promote effective resource utilization.
In this role, you will collaborate with a multidisciplinary healthcare team to develop comprehensive plans of care that address medical, psychosocial, and financial needs. You will work directly with patients, families, and external community resources to align care with patient goals, identifying barriers to outcomes and managing them proactively. Your day-to-day involves conducting comprehensive assessments, documenting patient responses, and making data-driven recommendations to modify care plans as needed.
Beyond direct patient care, you will contribute to departmental goals by participating in Performance and Service Improvement teams. You will assist in program evaluations by analyzing customer service surveys, length-of-stay data, and benchmark comparisons to ensure adherence to best practices and regulatory standards.
