CareConnectMD DCE is a specialized High Needs Direct Contracting Entity (DCE) dedicated to serving medically complex Medicare beneficiaries. About the Role
As a Care Manager RN or LVN, you will serve as the primary point of contact for patients throughout their treatment episodes, utilizing your clinical expertise to assess needs and coordinate care across diverse settings. You will work within a comprehensive interdisciplinary team to develop and implement person-centered plans of care that benefit the body, mind, and overall quality of life of frail patients with significant medical issues.
In this role, you will act as a vital liaison between patients, families, and various care settings, including hospitals, skilled nursing facilities, and home care. Your day-to-day involves monitoring patient utilization, facilitating smooth transitions of care, and providing education to caregivers. You will navigate an unstructured, start-up environment where initiative and the ability to work independently are essential. This position requires local travel to onsite facilities and availability for on-call duties, blending combined in-office work in Costa Mesa, CA, with remote flexibility.
